Driving for the College? Submit required information.
The college’s Fleet Safety Policy and insurance company requires annual review of motor vehicle records of faculty and staff who anticipate driving on behalf of the college during the current academic year. If you plan to drive a college vehicle, rent a vehicle, or use your own personal vehicle on behalf of the college, you must complete and submit the following information annually:
- Complete a Qualtrics survey to submit license information and review the safe operating procedures.
- Complete and submit signed motor vehicle record form to Carol McMillin, Corson 015C, for processing via campus mail by Friday, October 18. Please avoid sending the completed form through email.
